Quick start

Open the chart in your browser. Nothing to install, no account needed. A short walkthrough greets you on each visit; skip it whenever you like.

Click a cell in the row you want to fill. That single click does two things: it selects the measurement — pocket depth, recession, bleeding, plaque, suppuration, mobility or furcation — and it hands the keyboard to the chart.
- Type the value. The chart moves to the next site by itself, so you can keep your eyes on the probe. Enter and the arrow keys move you around without entering anything.
- Set tooth status where it differs. Click any tooth number to open its dropdown and mark it Present, Missing, Pontic or Implant. Navigation then skips the teeth that aren’t there.
Tip: click any tooth number to change its status (missing, implant, pontic).
- Watch the summary. Bleeding, plaque and suppuration percentages, mean pocket depth, the share of sites at 4 mm or more, and the deepest pocket all recalculate as you chart.

- Download the PDF before you leave. The free chart is saved nowhere unless you send it to a colleague — closing or reloading the tab loses it.
Tip: fill in Clinic Branding once before your first session and every export you produce afterwards carries your logo and contact details.
Reading the chart
Both arches and every surface sit on one page, so nothing you enter scrolls out of sight.
The summary counts every site with a pocket depth of 4 mm or more.
- Pocket depth is written in blue, recession in red, each inside a tinted cell in its own colour. There is no severity colour ramp — a depth of 8 mm is the same blue as a depth of 2 mm, and the summary is where severity is reported.
- A grey dash means no value has been recorded at that site yet.
- Bleeding, plaque and suppuration are filled dots — red, violet and amber. A hollow ring in the same colour means the site was checked and nothing was found, which is a different fact from never having looked.
- Mobility is one value per tooth, 0 to 3, recorded once at the buccal site rather than per site.
- Furcation is graded I to III and appears only on molars and upper premolars, the teeth that have a furcation to probe.
Keyboard and navigation
Typing goes to the chart itself, so if the keys don’t respond, click the chart once.

You can also click a bleeding, plaque or suppuration circle to toggle it straight away. Clicking a number cell moves you there without entering anything.
Left to itself, the chart walks the whole mouth in four passes:

- Upper jaw, buccal — 18 through 28.
- Upper jaw, palatal — 18 through 28.
- Lower jaw, buccal — 48 through 38.
- Lower jaw, lingual — 48 through 38.
Quadrant numbers follow FDI notation: Q1 upper right, Q2 upper left, Q3 lower left, Q4 lower right. Switching measurement keeps you on the same tooth whenever that site applies to it, so you can fill one parameter across the arch or work tooth by tooth — the chart doesn’t force either order on you.
If your team reads teeth as 1 to 32 rather than in FDI, switch the whole chart over:

Click a tooth number to open its dropdown.
- Missing and pontic teeth are greyed out and skipped by navigation, so your eye and the cursor both pass over them.
- Implants stay in the traversal and are charted like any other tooth — peri-implant probing uses the same six sites.
- Marking a tooth missing or pontic clears its measurements. If it already holds data you’ll be asked to confirm first.
Tabs and settings
- Charting holds the chart itself.
- Odontogram records caries, restorations and planned treatment, surface by surface. How the odontogram works.
- Patient Info takes the patient’s details and the reason for the visit. Every field is optional — the chart works with all of them blank.
- Clinic Branding takes your logo and contact details, which then appear on the export.
Help opens the full reference — the legend, every shortcut, and what the current measurement expects — without leaving the chart.
Export and share
Download PDF produces a two-page A4 record: the full chart on page one, the summary on page two, both carrying your branding and the patient’s details.
Behind the ⋯ menu:
- Send to a colleague turns the chart into a link they can open on a phone, and prints a QR code for it on your next PDF. It is the one action here that uploads a copy — how sharing works.
- Save Chart Data downloads the chart as a file you can reopen later with Load Chart Data — useful for carrying a chart between devices, or for picking up an examination you had to interrupt.
- Clear Chart wipes the measurements together with the patient and clinic fields.
Storage note: nothing is saved unless you use Send to a colleague. This chart lives only in this browser tab, so download the PDF or the chart file before you close it. If you’d rather charts were kept per patient and compared between visits, that is what an account adds.
