    Choose what you are recording

    Caries, filling, crown, root canal or extraction, each one chip on the palette above the chart.

    Choose existing, planned or completed

    Grey for what is already there, green for what you intend to do, blue for what you have done. A surface can carry more than one.

    Click the surface

    Each tooth is split into five zones. Crowns, root canals and extractions apply to the whole tooth, so the box becomes one target.

What it records

A restorative and treatment-planning chart, over the same teeth in the same order as the periodontal chart, and under whichever numbering you use — FDI or 1–32.

Five surfaces per tooth
Mesial, distal, buccal, and occlusal or incisal, plus lingual or palatal depending on the arch. The chart works out which is which from the tooth number.
Findings and treatment at once
Caries is drawn as a finding rather than a treatment, so it sits on top of a restoration instead of replacing it. An old filling you plan to replace shows as both.
The codes you bill under
Enter the code your practice bills each treatment under, once. They print on the PDF and travel with a shared chart.

How the odontogram works

Questions

Is it really free?
Yes. The odontogram, the periodontal chart and the PDF export are all free and need no account. A free account adds saved patients and progression across visits.
Does it come with treatment codes?
No, and that is deliberate. Fee schedules differ by country and most are licensed, so shipping one would be wrong for nearly everyone. You enter your own once and they are remembered in that browser.
Which tooth numbering does it use?
Both. A control above the chart switches between FDI and the 1 to 32 Universal system, and the PDF follows whichever is showing.
Can I record periodontal measurements too?
Yes. The odontogram is a tab in the same tool. Pocket depths, recession, bleeding, plaque, suppuration, mobility and furcation are on the charting tab, over the same teeth in the same order.
Is anything saved or sent anywhere?
No. What you chart stays in the page and a reload clears it. A chart is only ever uploaded if you press the button that creates a share link.
