Send a chart to a colleague

Hand a finished periodontal chart to another clinician as a link, or as a QR code printed on the PDF, so the referral arrives with the measurements attached rather than described.
- Opens on any phone, with no account and nothing to install.
- Your next PDF export prints a QR code for the link.
- What you send is a snapshot: editing the chart afterwards does not change what the recipient sees.
Sending
Open the ⋯ menu above the chart and choose Send to a colleague. A bar appears holding the link, with Send and Copy link beside it: the menu item creates the link, the bar hands it over.
The same item sits in the ⋯ menu of the free chart and of a patient's chart in an account.
Twenty links per hour, counted per internet connection. A practice usually shares one, so wait an hour if you are told there have been too many.
What travels with it
Every measurement and tooth status, plus patient name, clinic, practitioner and chart date.
- In the free chart, those four are whatever you typed on the Patient Info and Clinic Branding tabs. Leave them blank and the link carries the measurements alone.
- In an account, they come from the patient record and your profile. Your patients are already stored there; what sending adds is a link to one chart that opens without an account.
Date of birth, patient ID, the reason for the visit, clinical notes, your contact details and your logo are never sent.
The QR on the PDF
Share first, then export. The code prints only when a link already exists for the chart in front of you, in this browser tab. Two things drop it:
- You edited the chart after sharing — including the patient's name. The link still opens what you sent, so the code is left off rather than printed beside numbers it no longer matches. Send again to restore it.
- You reloaded the page. The link keeps working, but the tab has forgotten it.
Without a code the footer still carries perioapp.zermmi.com and a clickable link to the tool. The band prints on page two, under the summary.
What the recipient sees
The link opens at perioapp.zermmi.com/s/… on any phone. No account, no install.
The page shows the header, the six summary figures, and every tooth with something recorded, laid out for a phone rather than as the wide chart. It has a legend and an FDI / 1-32 switch. Nothing on it can be edited: Open this chart to edit loads the measurements into the tool on a wide screen, without the patient's name.
Withdrawing a link
Revoke appears on the share bar when you were signed in as you created the link. It stops the link opening, and tells anyone who follows it afterwards that the sender withdrew it. Use it before you close the bar — there is no list of past shares.
Signed out there is no Revoke, and the bar says so: an anonymous link cannot be withdrawn by anyone.
Sharing the same chart again creates a second link rather than updating the first. The first keeps working, so that a QR already printed on paper does not stop resolving.
Links do not expire, for the same reason — a QR on a referral may be scanned years later. Deleting your account deletes its links, and its printed codes stop working.
Storage note: in the free chart nothing is saved — Send to a colleague is the one action that uploads a copy. In an account your patients and charts are already saved; a share adds a link to one of them that opens without an account. The rest of the free tool is covered in the periodontal chart guide.
